This thing is always getting loose.  How can I keep it tight so it doesn't come undone?

Not enough information.

What brand and model of head? Got a photo of the plate (both sides)? Is there a washer? How are you tightening it? When you say loose, what precisely does that mean? I.e. what happens, and how are you using the head?

Sounds like you haven't tightened up the screws under the plate that secure the head to the plate...

Use a rubber plate between the plate and your camera.

To the OP... Have you fixed this issue ? Did you see the screws underneath the mounting plate of the tripod ?
