The smoke could be created by burning incense sticks or dhoop sticks. A external flash would definitely be required. As a work-around, you could try using another camera & fire its inbuilt flash. Hide the camera in the person's silhouette. Offcourse don't expect to get such a wide coverage with the inbuilt flash.
