Not sure what camera you use, but if you have mulitple focus points make sure that you are using the right one.
The T3i has multiple focus points that can be selected via a button and wheel OR you could use the center point, focus on the OH and half depress the shutter, move to frame, then complete the shutter press.The focus points in use will show red in your viewfinder.
