to a model in a shot you have taken, which will get published in a magazine?  This model was paid for their time during the photo shoot if that makes a difference.

You do not have to credit the model unless it's a provision of your model release (it shouldn't be).  You DO need a signed model release.

I think Bob's right, you'd probably need a release signed but I don't think a model credit is usually given.

How is the photo going to be used, in an article or an ad? For editorial use (an article) you may or may not need a release (so probably better to get one signed). I don't think the model would be named unless the article was about fashion or modeling, etc.

If it's an ad you'd definitely need a release to use someone's image for commercial work like advertising, but there usually doesn't seem to be a photo credit for the photographer or any sort of credit for the model for an ad.

Try http://asmp.org or PPA for info.
