1.  What is your ideal set up?
2.  Are you ever asking people before you take a photo that has them in it? 
3.  Are you ever getting release forms completed?

  1. If I were rich, an Olympus PEN-F.
  2. If the shot is close, and they are standing around, yes.  But that is not common, usually I take crowd shots, or the subject is just walking by.
  3. No.  My shots never see much of an audience.

2. It depends... if they're close enough I will. But that can also ruin a shot if people are trying to pose a certain way. Sometimes it's better to get a shot that's candid.
3. If I'm doing a singular person, yes. Groups or passerby, not like I should haha

I've never bothered, nor would I, to get a release form.

Nobody in a public place can have an expectation of privacy, it's that simple.
