Need help on ideal set up for photographing insects eyes?

I'm intrigued with these sort of shots and curious to know what is ideal gear and settings please.

If you could have anything (within reason) to photograph insect eyes, what would you need? 

Software?

Lens?

I'm guessing camera is anything decent from last few years?  

What about camera settings?

Flash?

The better setup would be a true macro lens in the 90-105mm range with additional illumination. I'm assuming you want to see an insect's eye facets ? Macro is my niche & I have loads of images on my Flickr stream. The light will allow you to stop down the lens for increased DOF (painfully shallow at macro distances) as well as stop motion (be it yours or your subjects). BTW AF & VR/IS/OS are of little to no value at higher magnifications. A longer macro lens as I suggested will put a little more distance between you & your subject making lighting easier to do.
