Photo log details while in the field?

Do any of you keep track of photo log details while you are out in the field?  AKA writing stuff down from the shot?  Are you using a notepad or have you found an app that works well?  I'm using a small notepad, but would like to see if any apps will do this for me?

Hi Adam

These days I use an app on my mobile phones,  such as the message draft option of the SMS-text app. 
I simply start a draft SMS message  but never send it  just save it .  It  lets me  type at least 600 characters of whatever info I want .
I  used to keep little stickie-note sheets  but  they  all got  torn , or lost ,or too faded or wrinkled and  eventually had to be thrown out .

I record pertinent information on a notepad. I do carry a phone, but when actually making a photograph it is turned off.

I have a cell phone App called "Notepad" that I frequently use for taking quick notes, but I also have a 3 X 5 top ring note pad in my camera bag that I use. If in bright Sunlight, when I can't see the cell phone screen, I usually resort to the paper notepad.

When doing video work with many short scenes I have a full size 8 1/2 X 11 notepad for taking notes, camera, subject, light positions and time of shoot, because there is usually too much information about the shots to try to cram into the small notepad or cell phone. I frequently have an assistant doing the recording of this information, so it's not always in my handwriting. I sometimes have an assistant with steno experience. She then types up the notes later for me, and this has worked great.  

I have also used my backup audio recorder, a Zoom H1N as a verbal note taker, when I want to record my personal thoughts quickly. Each recording becomes a separate file name, so it's easy to separate the notes from the backup audio later back in the studio. An H1N audio recorder is the size of a hand held microphone and records incredibly clear audio on a small Micro SDXC removeable memory card that can be removed and played back on a PC.
