Some questions about starting a business.

I am getting ready to really kick it into gear and start up my business.

1- What forms and info do you include in your client packages Is there a place with templates for these?

2- Do you have to have a small business license? Or does it only matter if you are making a certain amount of profit?

3- Print pricing.. what percentage do you mark up your prints? Do you prefer a la carte only of Packages as well?

I include a model release form. I have my client sign a contract. Just a simple contract stating I own the images. If they buy a cd, they need to have written permission from me to print etc..
Yes I have a business license.
I mark my prints up 100%

My tax guy suggested that I look into getting a tax ID (like an SSN), a business license from my locality and file as a Sole Proprietor. Check with your local Chamber of Commerce or small business unit in your local or state government. A tax adviser is also a good idea but don't wait very long, "tax season" is coming fast.

Shoot, learn and share. It will make you a better photographer.

Well I won't be filing as a business this tax season. I know I didn't make enough last year. However I do think I will start my business this year. Start 2011 off in a good year.
