I enjoy portraits so having a 18% card that cost me less than $9 bucks is by far the best money spent.  At the end, this helps me get my white balance and exposure nailed down.  Seeing that my camera has a handy dandy custom white balance setting (thank you Canon), I can use my 18% gray card kind of like a incident light meter and spot meter off it just prior to photographing my subject.

Lens cloth. My Nikons are in no need of white balance or exposure assistance as they nail it every time.....:P

Pretty much the same here. I have an Expodisc that I use very rarely now. I used it to setup my custom WB banks long ago on my D300. Some of which I have changed. But the polarizer is the best buy.

Spare battery gets the most use in my bag.

Instead of a simple grey card I use a color checker card (which includes a grey scale).  But - I reference this only in post, not when taking the shot.

I have a polarizing filter and hardly ever use it.

These days I do a lot of out door shooting and my favorite tool is my Hoodman Loop.  Super handy on sunny days.

Depends on the shoot, however I will say my bounce cards, micro fiber cloths and travel tripod get used a lot.
